Also known as autologous chondrocyte implantation, ACI surgery is a procedure used to treat cartilage damage in the knee. Cartilage is the tissue that covers the ends of the bones in the joint. It is a flexible but tough tissue that helps cushion the joint and acts as shock absorber.

ACI surgery is a procedure that takes a biopsy of healthy articular cartilage then grows these cells in a lab. Once this cartilage has been grown it is reimplanted into the knee. This procedure is performed with an open incision. During the surgery, the kneecap is moved in order to give access to the inside of the knee.  A second incision is then made over the shin and tissue is taken from the shin before being implanted over the damaged cartilage. Once this is done, the lab-grown cartilage is injected beneath the implanted tissue from the shinbone.

Because the recovery period can be lengthy and require serious effort from the patient, ACI surgery may not be the ideal solution for everyone. Patients who undergo this procedure must be willing and ready to pursue rigorous rehabilitation through physical therapy.

In addition to a willingness to undergo rehabilitation, patients who choose to have ACI surgery should also meet a number of other important criteria. Chief among these criteria is that the area of cartilage damage be of a relatively small size. Patients with broader damage or arthritis are not generally thought of as the best candidates.
